Missouri Department of Corrections (DOC) Medical Neglect Reporting Form
This form allows family members to report instances where the Missouri DOC has failed to provide necessary medical care, withheld essential medications, or denied critical treatments to incarcerated individuals. Submissions should include specific details such as the individual's name, facility location, medical condition, denied treatment, and any previous attempts to address the issue. Reports will be reviewed to advocate for appropriate medical care and hold the DOC accountable for ensuring the health and well-being of those in their custody.
Missouri Department of Corrections (DOC) Excessive Use of Force Reporting Form
This form allows family members to report incidents of excessive or unlawful use of force against incarcerated individuals within the Missouri DOC. Reports should include details such as the individual's name, facility location, date and description of the incident, injuries sustained, and any witnesses. Submissions will be reviewed to document patterns of abuse, advocate for accountability, and protect the rights and safety of those in custody.
Missouri Department of Corrections (DOC) Harassment, Intimidation, Retaliation, or Discrimination Reporting Form
This form allows family members to report incidents of harassment, intimidation, retaliation, or discrimination against incarcerated individuals within the Missouri DOC. Reports should include details such as the individual's name, facility location, date and description of the incident, individuals involved, and any threats or punitive actions taken. Submissions will be reviewed to document patterns of misconduct, advocate for accountability, and protect the rights and safety of those in custody.
Missouri Department of Corrections (DOC) IRR/HSR Form Denial Reporting Form
This form allows family members to report instances where the Missouri DOC has wrongfully denied, ignored, or failed to process an Informal Resolution Request (IRR) or Health Services Request (HSR) submitted by an incarcerated individual. Reports should include details such as the individual's name, facility location, date of submission, the reason for the request, and any responses or lack thereof from DOC staff.
